Job Summary:
We are a long-term care home health agency that provides home health services through Primary Home Care (PHC) program which provides non-technical, non-skilled, in-home attendant services to people who have an approved medical need for assistance with personal care tasks.

Job Responsibilities:

• Supervision of personal care attendant, new client intake, visit maintenance, and care with the caregiver and client. Provide care to a client when an appropriate provider cannot be located.
• Directs and coordinates care; assumes responsibility for quality, and safety of services delivered in compliance with State and federal regulations
• Ensures timeliness and completeness of all documentation. Able to multi-task.
• Monitors systems and identifies problem areas to Administrator both verbally and through written reports
• Uses effective interpersonal relations and communication skills; facilitates the use of these skills by other team members to achieve desirable outcomes.

Job Qualifications:

Education: High School Diploma, Bachelors preferred

Experience: Entry level and at least one year of home-health setting.

Skills: Excellent interpersonal skills and ability to communicate effectively. Demonstrates proven decision-making skills. Must read, write and comprehend English.

General knowledge of office equipment such as a computer, copier, telephone, etc., ability to provide excellent customer service, have great communication and organizational skills. Team player.

Proficiency with Vietnamese and Cantonese is a plus. Able to drive.
